· Income verification: Please provide two paycheck stubs, a lease contract, and electric, gas, water, phone, car, and insurance bills to be totaled for budget assessments.

o Without kids: Total net income after taxes should be under $27,000, but we will provide services on a case-by-case basis.

o With kids: Total net income after taxes should be under $33,000, but we will go provide services on a case-by-case basis.

· Background check for applicant and spouse/co-parent if applicable

· Drug test, weapons check, psychological assessment, and evaluation of living conditions for applicant and spouse/co-parent if applicable

· Qualifications will be assessed by the registration team and all documents will only be used for the qualification process then will be securely shredded. Submit all documents through our registration form.

· Family support: please explain the level of family support you currently have

· Client needs: please explain what your current needs are and what services you’d like us to provide for you
